The Medical Benefits Settlement will provide for the following benefits: 1) Compensation for Specified Physical Conditions; 2) Participation in the Periodic Medical Consultation Program; and/or 3) Participation in the Back End Litigation option for a Later-Manifested Physical Condition. A description of these benefits can be found in the Court-directed Notice available on this website.

If you are a Medical Class Member, you must complete and submit forms to request benefits. To request Specified Physical Condition and/or Periodic Medical Consultation Program benefits, you must complete and submit a Proof of Claim Form. To participate in the Back End Litigation option for a Later-Manifested Physical Condition, you must submit a Notice of Intent to Sue Form.

Once you have completed a form you have downloaded from the website or requested via mail, you must mail it to the address available on the Contact Us page.
