How to Revoke an Opt-Out
The deadline to revoke requests for exclusion from the Medical Benefits Settlement Class was January 11, 2013, the date the United States District Court for the Eastern District of Louisiana granted final approval to the Medical Benefits Class Action Settlement. However, the Medical Benefits Settlement Agreement allows for revocation after this date with BP’s consent, which shall not be unreasonably withheld.
Pursuant to the Court’s order entered on November 16, 2012, a person who submits an opt-out revocation after entry of a Final Order and Judgment must withdraw any pending objection, and waive any and all current or future objection, to approval of the Medical Benefits Class Action Settlement.
If you would like to revoke your opt out request, you may request BP’s consent by completing the Revocation Request Form and returning it to the following address:

You may also submit a written request to the above address stating “I wish to revoke my request to be excluded from the Medical Benefits Settlement Class” (or substantially similar clear and unambiguous language), and also provide with your submission:
- Printed Name
- Phone Number
- Date of Birth
The revocation request must be personally signed by you, not by your attorney. You may not submit your request by phone, by email or on the website.
Your request will be forwarded to BP, and you will receive a response in writing as to whether BP agrees to accept your request.
If you choose to revoke your opt out request, and BP consents to the revocation, this does not mean that you are automatically a member of the Medical Benefits Settlement Class or are otherwise eligible for benefits under the Settlement. These determinations are made under the terms of the Medical Benefits Settlement Agreement. It does mean, however, that the Medical Claims Administrator will review your claim if you timely submit a Proof of Claim Form.
